Your First Appointment

Congratulations! You’ve now taken the first step in monitoring your hearing health by making an appointment at one of our locations nearest to you. At your first appointment you will be asked to fill out some simple forms to give our audiologists an overall look at your medical history and hearing issues. If you would like, these forms can be emailed to you to fill out prior to your appointment.

Please be sure to bring the following to your appointment:

  • Your health insurance card(s) and copayment.
  • Your referral or prescription for your hearing test (if applicable and required by your insurance). Referrals and prescriptions may also be faxed to our office.
  • A list of your medications, including dosage information.

After your new patient paperwork is completed you will meet with one of our doctors of audiology. If you have scheduled a test, your hearing will be evaluated in one of our sound booths. After the test, the audiologist will speak with you about your test results and discuss potential treatment options if needed.

We recommend that you bring a significant other with you, especially if you have been having trouble hearing and/or understanding that person’s voice.
You will be able to try out various hearing technology options that day or take your time to think about it. What matters most is that you are comfortable with whatever decision you make. The appointment will take about one hour.
